At Pain Medicine Consultants, we want to stay connected with you between visits - not just on appointment days. That is why we have introduced a Remote Therapeutic Monitoring (RTM) program through Pilothouse Health, designed to give our care team a fuller picture of how you are doing day to day, not just when you are sitting in our office.
Chronic pain and musculoskeletal conditions change between appointments in ways that are difficult to capture in a single visit. A patient may be doing well when they come in, but struggling significantly in the days or weeks before or after. Without a way to monitor that between-visit experience, our team is limited to a point-in-time snapshot. RTM changes that by creating a continuous connection between you and your care team throughout your treatment journey.
What Is Remote Therapeutic Monitoring?
Remote Therapeutic Monitoring is a structured program that allows our care team to stay connected with patients between office visits. Through short, regular check-ins completed from your phone, tablet, or computer, you share how you are doing across five key indicators. That data flows directly to our clinical team, giving them a much fuller picture of your health between appointments.
The program is completely free to you as a patient. For Medicare patients it is covered as part of your ongoing care. If you are on a private insurance plan, please check with our billing team who will be happy to confirm your coverage.
What Does the Check-In Look Like?
Each check-in consists of five simple sliders:
- Pain level (0-10)
- Mood (0-10)
- Activity level (0-10)
- Sleep (hours)
- Overall improvement (0-10)
The whole thing takes less than a minute. We ask for a minimum of one check-in per week, and you can log up to once per day if you wish. The more regularly you check in, the more useful the data is for your care team - but even a single weekly log makes a meaningful difference.
For patients who want to share more detail, there is an optional expanded section just below the submit button where you can add information about your pain type, the time of day when pain is worst, duration, and anything else you want your care team to know. It is completely optional and never gets in the way of patients who just want to complete the five sliders and move on.
There is no app to download. The platform works on any phone, tablet, or computer - just open your browser and log in.
What Does Your Care Team See?
Every check-in you submit goes directly into a secure, HIPAA-compliant clinical dashboard that your providers and staff can access at any time - between and before appointments. They can see how your pain, mood, activity, sleep and overall improvement have been trending over time, not just how you felt on the day of your last visit.
If something changes between appointments - a significant spike in pain, a sustained drop in activity, or a pattern that warrants attention - your care team can reach out proactively rather than waiting until you come in. This kind of early intervention is one of the most meaningful benefits of the program, and it is only possible because we have continuous data rather than a single point-in-time assessment.
Why It Matters for Your Care
The goal of Remote Therapeutic Monitoring is simple - better informed care at every appointment, and more support between them.
When your provider reviews your trend data before a visit, they walk in knowing what has been happening over the past weeks rather than starting from scratch. They can ask more targeted questions, validate what you have been experiencing with real data, and adjust your care plan based on a complete picture of your health journey.
Patients consistently tell us they feel more heard and more connected to their care team when they are part of an RTM program. That sense of continuity - knowing that someone is paying attention even between appointments - makes a real difference to the experience of managing a chronic condition.
The program is delivered through Pilothouse Health, a fully managed Remote Therapeutic Monitoring platform built specifically to help medical practices stay connected with their patients between visits.
